You may, at your discretion, grant 麻豆APP permission to release information about your student records to individuals or agencies by submitting a completed Authorization to Release Student Information form.
Open the Authorization To Release Student Information Form.听Print it, complete all the necessary fields, sign it, then submit it to Enrollment Services.听Signed forms and required documentation can be uploaded to your eStorm Student Center by clicking on the Manage Uploaded Documents link.
